Soldier Family Medical Center (SFMC) is excited to partner with our Active Duty, Retiree, and Family members to provide quality care for all your non-urgent, readiness, primary and preventative health care. You are very important to us and we value your input as we navigate 21st century medical care. SFMC is a Soldier Centered Medical Home (SCMH) and a Family Centered Medical Home (PCMH); Combined the clinic is comprised of 3 teams: Team Garrison, Team Family, and Team 11th BDE.
The Soldier and Family Medicine and Primary Care Clinic offer the following services:
Lab: Monday-Wednesday
7:00 a.m. - 4:00 p.m. (Last Check in will be at 3:45 p.m.)
Thursday (Training Day)
7:00 a.m. - 2:00 p.m. (Last Check in will be at 1:45 p.m.)
Friday
7:00 a.m. - 3:30 p.m. (Last Check in will be at 3:30 p.m.)
Labs will be closed on federal holidays.
Radiology: Monday-Friday 0700-1200 and 1300-1600 (Appointment/Walk-in)
Physical Therapy: Monday-Friday 0700-1145 and 1245-1600 (Appointment Only)
Audiology: Monday-Friday 0720-1150 and 1250-1500 (Appointment Only)
TBI: Monday-Friday 0730-1200 and 1300-1600 (Appointment Only)
Pharmacy: Closed
Integrated Behavioral Health: Monday-Friday 0700-1200 and 1300-1600 (Appointment Only)
Immunizations: Monday – Friday 0730-1100 and 1300-1500. SFMC enrolled patients (Appointment/Walk-in)

Family Care Mission Statement
To provide patient centered focused care with a multidisciplinary approach; allowing for the patient to collaborate and coordinate with their primary care provider and nurses for their health. Family Care consist of 7 Providers, 3 RN’s, and 7 LVN’s. This core team works to take care of new health concerns as they arise, ensure delivery of preventive screening and services, manage chronic health problems, and promote a spirit of health and wellness. The relationship between the patient and the health care team is the heart of the Patient Centered Medical Home (PCMH) and is designed as the essential principle of putting patients first.

Soldier Care Mission Statement
Soldier Family Medical Center provides healthcare to Fort Bliss Soldiers. The clinic is a Soldier Centered Medical Home (SCMH) comprised of both FORSCOM and MEDCOM providers that deliver comprehensive primary care and enhanced medical readiness. In this SCMH, you will collaborate with you primary care provider, nurses and medics to take responsibility for your health and care coordination. This core team works with you over time to take care of new health concerns as they arise, ensure delivery of preventive screening and services, manage chronic health problems, and promote a spirit of health and wellness. The SCMH strives to reduce injuries, improve health and longevity and facilitate rapid return-to-duty to optimize combat readiness.
